End of life decisions are not easy. We are often asked, “When
should I consider hospice care?” Patients and family
members should discuss hospice care as an option whenever
someone is facing a life-threatening illness and curative
care is no longer an effective option.

Hospices' staff understands that the decision to stop
curative care can stressful and difficult. We
are available to discuss any concerns with the patient,
family and physician.
Our admissions team consists of a
registered nurse, admission coordinator and social worker.
Anyone can make a referral for hospice care, including
a patient, family member or physician.
Once a referral is made, our admissions
team will contact the patient and/or family and arrange
for an appointment in order to assess the patient’s
condition and discuss hospice services and patient
and family rights. If a physician has verified appropriateness
and the patient wishes to begin hospice services, the
patient will be asked to sign a consent form stating
that he or she understands that the care is palliative
(aimed at pain relief and symptom control) rather than
curative.
