At Community Hospices we provide a wide range of hospice services to address our patient’s medical, psychological, spiritual and social needs. Our services provide terminally ill patients and their family members with the highest-quality end of life care, with special emphasis on pain control and symptom management so that patients may live with dignity and comfort.

Our hospice staff is on call 24 hours a day to answer questions and provide assistance. We provide services, based on patient and medical needs, seven days a week, 365 days a year. Anyone can make a referral, including patients, physicians or family members. To make a referral simply call our admissions office at (866) 234-7742 and our staff will walk you through the process.
